The Public Health etc. (Scotland) Act 2008 
The Public Health etc. (Scotland) Act 2008 provides a statutory framework for public health action to protect the people of Scotland from infectious disease, contamination and other such hazards.

Notifiable Diseases, Notifiable Organisms and Health Risk States
Part 2 (Notifiable Diseases, Notifiable Organisms and Health Risk States) of the Act came into effect on 1 January 2010.
To support the notifications process and enable those Registered Medical Practitioners working in secondary care who do not have access to SCI Gateway to send notifications to their NHS Board, HPS has provided an alternative notifications form to be used from 1 January 2010.
For those Registered Medical Practitioners working in primary care, all written notifications with effect from 1 January 2010 should be undertaken electronically via SCI Gateway. Those Registered Medical Practitioners working in secondary care who have access to SCI Gateway are expected to notify electronically via SCI Gateway from 1 January 2010.

Methods of Completion and Transmission:
The form can be completed and transmitted in two ways:
- Secure email: complete it electronically and send by secure email to the Health Protection Team in your NHS Board.
OR
- Secure post/fax: those without ready access to a computer can complete a paper copy of the form and send by secure post or fax to the Health Protection Team in your NHS Board.

Contact details of all NHS Board Health Protection Teams can be accessed on the last page of the Scottish Government implementation guidance on Part 2 of the Act. Enquiries in relation to the completion of the form should be addressed in the first instance to the Health Protection Team.
