To help reduce HAIs in NHSScotland HPS has produced a number of tools that when implemented may increase quality improvement.

Bundles

A number of steps when performed collectively, reliably and continuously can improve patient outcomes.

Key recommendations

Key recommendations have been produced from the literature reviews. These should be included in Quality Improvement Tools.

A summary of the key recommendations are presented in the form of a diagram which can be printed off and used in practice.

Cause and Effect Charts

A Cause and Effect (CE) chart, also sometimes called a 'fishbone' diagram, is a tool for discovering all the possible causes for a particular efffect. The major purpose of the CE chart is to act as a first step in problem solving by generating a comprehensive list of possible causes. It can lead to immediate identification of major causes and point to the potential remedial actions or, failing this, it may indicate the best potential areas for further exploration and analysis.

Anyone in the clinical setting with the agreement of the clinical team and Quality Improvement Leads can use the cause and effect charts.

Literature review

Review of evidence base and expert opinion to ensure key recommendations are the most critical for improving and streamlining practices.

An MHRA alert has also been issued on the use of Risk of anaphylactic reaction due to chlorhexidine allergy.
